New data tool to visualise primary care data in Wales
Data sets

Public Health Wales have launched a new Primary Care Clusters profiling tool to help stakeholders understand and utilise primary care cluster data better. Providing users a summary profile, the tool will allow users to visualise key indicators for all clusters within a health board simultaneously. Users can compared data with both the health board and across Wales.

The tool includes a brief narrative highlighting key findings from the data in both English and Welsh. Launched yesterday (27th March 2025), you can find it here.

A primary care cluster promises local health and care services within a geographical area, such as GP practices, dentists, and community pharmacies. There are over 60 clusters in Wales, each one serving around 30,000 to 35,000 people.

The tool presents 15 key health indicators for each cluster, and covers areas like deprivation, chronic conditions, and emergency clusters. With the tool, users can quickly identify clusters with high or low rates for these indicators compared to either the health board or the national average.

The tool has made these insights more accessible for health professionals, public health experts, financial planners, policymakers, and their stakeholders, and will help them target inventions more effectively, helping to improve population health overall.

For each health board, key findings accompany the data, developed in collaboration with the Primary Care Division at Public Health Wales.

Nathan Lester, head of the analysis team at Public Health Wales, said: “The new Primary Care Clusters profiling tool is an exciting step forward in our commitment to providing high-quality, insightful data for healthcare professionals in Wales. Comparing indicators such as mortality and disease prevalence across different clusters is now easier than ever before. This tool provides a vital resource to guide efforts to improve population health and reduce inequalities.
